# A name to ask for. A record to check.

Write down who would treat you, what medicine they offer, and which pharmacy would send it. Use the checks below for each name.

A practical next step

## “How can I check your licence?”

Ask for the person’s full name and licence type. The title beside the name helps you choose the right search.

Keep the name and the licence type in the same note.

The person providing care

## Where to check an Arizona licence

MD and DO are types of doctor; each has an Arizona board, a state body that checks licences and follows up on complaints.

These Arizona search routes were recorded as of October 3, 2026.

### A doctor with an MD title

The Medical Board’s Doctor Search shows licence details and public board orders, which are official decisions about the doctor’s licence or conduct.

### A doctor with a DO title

The Osteopathic Board has its own Find your DO page, which links to its doctor search.

### A nurse practitioner

Arizona’s Nursing Board lets you check a nurse’s licence and certificate for work beyond general nursing, such as nurse practitioner care.

As of October 3, 2026, the Nursing Board must also grant a nurse practitioner permission to prescribe within the nurse’s training.

Ask to check both records. Also ask whether the board allows that nurse to prescribe the medicine being discussed.

### A physician assistant

Physician assistants, also called PAs, have a separate Arizona board with a PA Search and licence verification.

### “What if the record looks clear?”

As of October 6, 2026, some licence-board records are restricted from online display in Arizona; further public records can be requested.

A blank online section about action by a board does not prove there were no past actions or complaints.

The place handling the medicine

## “Who will prepare and ship the medicine?”

Write down the pharmacy’s exact name. Have its permit confirmed by the Arizona State Board of Pharmacy.

As of October 3, 2026, a pharmacy based outside Arizona must hold this state’s permit before shipping medicine into Arizona.

A current permit to operate as a pharmacy, or equivalent permission, is also required where that pharmacy is based.

A permit alone does not show that a medicine made to order meets state and federal rules.

The Food and Drug Administration (FDA), the U.S. medicines agency, lists state links for checking pharmacy licences.

The claim about the product

## “Does this code prove approval?”

A government drug directory uses the National Drug Code, shortened to NDC, to identify a product.

As of September 26, 2026, FDA says that having an NDC number is no proof of approval for a product.

As of September 26, 2026, a place that makes medicines can be registered with FDA without FDA approving that place or its medicines.

Ask to see the FDA record for the exact medicine being offered. Can the person treating you show the product’s name, its form such as a pill or shot, and the use it is approved for? If it is made to order, ask why that is being considered for you.

If something goes wrong

## Where to find help

### Severe allergic symptoms need urgent help.

MedlinePlus advises calling 911 for a severe allergic reaction, including a swollen face or tongue or difficulty breathing.

As of September 17, 2026, FDA’s MedWatch accepts reports of problems with medicines from patients as well as health professionals.

FDA’s public list of reported medicine problems is called the Adverse Event Monitoring System (AEMS), formerly FAERS.

A problem report on that list is not proof that the medicine was the cause.

As of October 3, 2026, Arizona’s Attorney General takes complaints from consumers about practices that deceive them or treat them unfairly.

## Words you may hear

Plain words: on

### The doctor’s kind of licence

MDs and DOs are checked through separate Arizona boards.

### A report of an unwanted problem

The report alone does not prove that the medicine caused it.
