Write down the pharmacy’s exact name. Have its permit confirmed by the Arizona State Board of Pharmacy.
As of October 3, 2026, a pharmacy based outside Arizona must hold this state’s permit before shipping medicine into Arizona.
A current permit to operate as a pharmacy, or equivalent permission, is also required where that pharmacy is based.
A permit alone does not show that a medicine made to order meets state and federal rules.
The Food and Drug Administration (FDA), the U.S. medicines agency, lists state links for checking pharmacy licences.

Ask to see the FDA record for the exact medicine being offered. Can the person treating you show the product’s name, its form such as a pill or shot, and the use it is approved for? If it is made to order, ask why that is being considered for you.
If something goes wrong
As of September 17, 2026, FDA’s MedWatch accepts reports of problems with medicines from patients as well as health professionals.
FDA’s public list of reported medicine problems is called the Adverse Event Monitoring System (AEMS), formerly FAERS.
A problem report on that list is not proof that the medicine was the cause.
